Make a Report

It is not your role to verify that abuse is occurring, only to alert others of your suspicions. Reporters may remain anonymous and cannot be held liable in civil or criminal court when reports are made in good faith.

To make a report, call Protective Services for the Elderly at the Department of Social Services during business hours at the toll-free line: 1-888-385-4225.  After business hours or on weekends or state holidays, please call Infoline at 2-1-1.  If you are calling from outside Connecticut, call Infoline 24/7 at 1-800-203-1234. You may FAX the W-675 form to 860-424-5091. However, telephone reports are preferred.

If the older adult in question lives in a long term care facility, then mandated reporters must follow procedures and complete the “Mandated Reporter Form for Long Term Care Facilities” (w-410) and fax it to the Department of Social Services, Social Work at (860) 424-5091.
